Thank you for registering your child for our SBC Oral Run 1 with Ms Shirleen!
To ensure a smooth start, please go through this Intake & Info Form thoroughly. Do note that every class/programme we run under Sowing Seedz has its own Intake & Info Form, hence you will have to ensure that you have been given the link to fill up the form for every programme you register your child for.

In Sowing Seedz, we aim to bring joy and surprise to your child throughout the year. To make this possible, we kindly request you to provide your current address in full below. By giving us access to this information, we will be able to send over some delightful gifts to your child on special occasions and holidays.
In compliance with the Personal Data Protection Act (PDPA), we do not retain addresses from year to year. This means that even if your child has been a student with us for quite some time, we will need you to provide your address each year. This policy ensures that we handle your personal information with the utmost care and privacy.
Please note that we strictly adhere to the submissions of this form. Therefore, we will not be able to sent over any gifts if this form is not filled up promptly or the addresses are incomplete/inaccurate. We do not follow up on missing submissions.
